​Youth Basketball Training – Participation Waiver & Liability Release
​Program Description: King of Bounce Training provides age-appropriate youth basketball training, motor skill development, and physical activity for children ages 2 and up. Sessions are safely conducted on-site at your child’s daycare facility.

​2. Acknowledgment of Risk & Insurance Coverage
​I, the undersigned parent or legal guardian, understand that participation in sports training
involves physical activity and movement. While King of Bounce Training takes every precaution
to ensure a safe, positive, and structured environment, there is an inherent risk of minor injury
(such as bumps, bruises, or scrapes) during physical play.
​Notice of Insurance: King of Bounce Training maintains comprehensive professional liability
insurance that covers participating children in the event of an accidental injury sustained directly
during a King of Bounce Training session.
​3. Release of Liability & Hold Harmless
​By signing below, I expressly agree to the following terms regarding liability:
1. Daycare Facility Exemption: I explicitly agree that the host daycare facility, its
owners, directors, and staff hold zero liability and shall not be held responsible
for any injuries, damages, or claims arising from my child’s voluntary participation
in King of Bounce Training activities.
2. Assumption of Responsibility: I acknowledge that this program is a voluntary,
extracurricular activity separate from the standard daycare curriculum. Any
claims or incidents related directly to the basketball training sessions will be
handled exclusively through King of Bounce Training and its insurance provider.
3. Medical Authorization: In the event of a medical emergency during a session, I
authorize King of Bounce Training staff to administer basic first aid and/or secure
professional medical treatment for my child if a parent or emergency contact
cannot be reached immediately.
​4. Photo & Video Release (Optional)
​I grant King of Bounce Training permission to use photographs and/or video footage of my child
taken during training sessions for promotional, social media, or marketing purposes. No personal
identifying information or names will ever be published with the photos.
​5. Parent/Guardian Signature
